I seem to recall from an article on this incident that I read some time ago that the gym's system was really insecure (like, anyone accessing the right URL could mess with it levels of insecure.)

The person giving the order to the software tool without fully understanding its capabilities and inability to weigh actions ethically is responsible. That person may or may not also be the owner of the device it was running on.
Technically correct: food standards can include numbers for "allowable" amounts of rat droppings and similar in some foods. Here's the FDA's version.

Thank $DEITY for that. I was afraid that the patient was going to have to avoid people who might say "Ignore all previous instructions and [do thing]" for the rest of his life. (Well, not really, but you get the idea.)
"Becoming"? It has been for months now.
Given the US, I'd bet 20 states say that it's illegal (but can't seem to enforce that unless one of them is New York or California), 20 states say that it's specifically legal (including Texas and Florida), and the remainder either haven't made up their minds yet or no one cares what they think because they don't have the population or economic clout to make anyone listen.
Has anyone done a study yet to see how many managers' insecurities about remote work can be handled by placing mannequins in chairs at the office?
Maybe they use a film camera. Good luck obtaining access to that by phishing.
Some cats are gobblers, some are grazers. Currently we have two gobblers, and one obligate grazer who throws up if she eats more than a tablespoon or so of food at a time. The logistics involved in making sure she gets enough without the other two stealing her food are . . . tedious.
The gobbler tendency may show up more often in rescue cats, who are likely to have been through rough patches here and there—we've never taken in a cat who was less than eight months old, so their habits are set before they get to us. Or it may just be a genetic quirk.

The system is broken by design. Notice-and-notice is much saner than notice-and-takedown, but would have given the big media mills the vapours.
