Random files appearing in the downloads folder on my phone has only happened once

Let me guess, it was an iPhone? Someone just used AirDrop and you might've allowed everyone sending you files.
It was Android :-) WiFi, NFC and BT turned off. It was Android 12 I believe, back in 2022.
That's actually a bit frightening and the feeling of being violated would linger for a while.
